Living well with a disability means different things to different people. I start by understanding what matters most to you.
I work with adults living with acquired injury, psychosocial conditions, progressive conditions, lifelong disability, and complex needs. I work with NDIS participants who are plan managed and self managed, DVA clients, and people accessing occupational therapy privately or through insurance.
My role is to assess where you're at, understand your goals, and work with you and your team to make the practical changes that will make the biggest difference.
Who I work with:
I support adults who are:
NDIS participants: newly entering the scheme, at plan review, or working towards specific goals. I can also support you to submit an NDIS access request.
Living with disability: including diagnoses such as acquired brain injury, amputation, stroke, neurological conditions, arthritis, progressive conditions, autism, intellectual disability, psychosocial disability, and complex needs.
DVA: Gold or White Card (condition specific) holders.
Insurance: Recovering from injury through workers compensation or CTP.
I accept referrals from support coordinators, LACs, GPs, specialists, case managers, and individuals directly
What I offer:
Functional Capacity Assessment (FCA): A comprehensive assessment of your capacity across physical, cognitive, and daily living domains. Used for NDIS planning, support needs determination, plan reviews, and identifying what goals to work toward. You receive a detailed written report with clear findings and recommendations aligned to the NDIS Reasonable & Necessary criteria. If you are seeking NDIS access, I can work with you to complete the required assessment and paperwork to assist you in your initial access request.
Activities of Daily Living (ADL) assessment: A structured look at how you're managing everyday tasks including self-care, home management, cooking, community access, and more. I work with you to identify your strengths, what's difficult, and what support, strategies, or equipment could help.
Assistive technology assessment & prescription: I assess your needs, trial options, and identify the right equipment with you, whether that's a mobility aid, adaptive tools for daily tasks, specialised seating, or something else. I manage the full prescription and funding justification process from assessment through to recommendation report, and can follow up with the NDIS directly as required.
Minor home modification assessment: An assessment of your home environment to identify minor modifications that would improve your safety, independence, and ability to manage daily tasks such as grab rails, ramp access, and minor bathroom adaptations. This service includes a written report and recommendations suitable for NDIS or other funding applications. Please note that I do not assess complex home modifications, including those that may require structural changes to your home.
Case management & care coordination: Coordination with your multidisciplinary support team. This includes connecting occupational therapy goals with your support workers, other allied health providers, and NDIS plan to ensure everything is working in the same direction.
Funding pathways:
NDIS:
I accept plan managed and self managed NDIS referrals.
Referrals are accepted from participants and their supports directly, support coordinators, local area coordinators, plan managers, and GPs and treatment teams.
Occupational therapy services are funded as a capacity building support under the Improved Daily Living section of your plan.
Other funding:
Private (self-funded): No referral required. Some private health funds cover occupational therapy as part of their extras. Check your policy for eligibility.
Medicare: Subsidised sessions can be accessed via a GP Chronic Disease Management Plan. This cannot be used for report writing.
DVA: Gold Card and White Card (for eligible conditions) are accepted following referral from your GP.
Workers compensation / CTP: Insurer-approved referral through your case manager or solicitor.
